Summary:Provides case management and follow-up of all known TB clients to ensure timely and appropriate treatment. Collaborates with local physicians, hospitals, substance abuse centers, correctional facilities and community organizations to promote TB education and insure continuity of care. This position works with a team of communicable disease specialists and outreach workers to assist with the implementation of patient care.Must be willing to train others. Adheres to HIPAA regulations. Essential Functions include, but not limited to: Identifies high risk populations and tailor interventions based on the needs of the population and the resources available. Provides treatments, administers medication/vaccines and provides preventive and maintenance care to patient/families consistent with care plan and/or physician treatment plan in accordance with nurse protocols, policies and/or procedures.Conducts comprehensive assessments of client’s health care needs. Conducts community needs assessment. Develops and sustains community partnerships.
Coordinates the refugee and immigrant programs.
Minimum Qualifications:Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
Preferred Qualifications*: *Bachelor’s degree in nursing. Professional experience in public health nursing. Experience in the specialized area of assignment. Excellent communication and customer service skills. Some public health and infectious disease experience is helpful. The ability to translate English to Spanish and Spanish to English orally and in writing is preferred.
Salary Information: $ 48,600 annual / Total compensation package, including leave value $87,256 / Pay Grade K
